Applicants have 12 months to submit all required documents for each primary service, except for the NY Certification Process in which the documents are collected differently. If the required documentation has not been provided to the FCCPT by the appropriate source within this time frame, the file will close.

The file may be reactivated by applying online through the same file number. There is a fee of U.S. $210.00 for all re-activations. The FCCPT recommends that a file not be reactivated until all of the requirements can be fulfilled.


Special instructions for re-opening the New York Verification Process      

Applicants who have ordered a New York Verification Process are not eligible for re-activation.

For the New York Verification process, FCCPT will make four requests to an educational institution or licensing/registration authority, approximately every 60 days. If, at the end of 240 days, there is no response from the institution or licensing/registration authority and FCCPT has completed its four requests, the service will be deemed complete, and the file will be forwarded to NYSED as unable to authenticate.

If an applicant wants to re-open the file and continue to have FCCPT send the request letters to the educational institution or licensing/registration authority, a new New York Verification Process will need to be ordered. Applicants will not be able to order this second service online. Instead, an applicant must fax or mail a letter to request to add this second New York Verification Process to their online file. The letter must include the applicant’s full name, file number, and signature. The fax number for FCCPT is 703-684-8715.

The applicant will need to submit new request forms for all institutions that did not provide the documents in the initial process to FCCPT.
